چکیده

Background: In previous reports, the patients are described to have transient symptoms from physiologic rotation or extension of the cervical spine, resulting from a cervical osteophyte compressing the vertebral artery and causing vertebral artery insufficiency, known as Bow Hunter syndrome. Methods: An 85-year-old female presented with new onset occipital headaches, nausea, vomiting and vertigo that were not precipitated by change in head position. Patient had bilateral cerebellar infracts. Patient underwent decompression and instrumented stabilization of the cervical spine from the posterior approach. Results: At one-year follow-up, patient remained stroke free with patent vertebral artery. Conclusion: To our knowledge, this is the first report of bilateral infraction from a vertebral artery insufficiency caused by cervical osteophytes without history of transient symptoms from movement of the head or neck, a variant of the Bow Hunter syndrome. Anterior versus posterior approach for vertebral artery insufficiency from osteophytic compression should be primarily based on location of the pathology and not the cervical level of involvement. *Corresponding author: Ripul R Panchal, Department of Neurological Surgery, University of California, Davis School of Medicine, 4860 Y Street, Suite 3740, Sacramento, CA 95817, USA, Tel: 916-734-3658; Fax: 916-452-2580; E-mail: ripul. Congenital Bilateral C2 Transverse Foramina Stenosis Causing Adult-Onset Vertebrobasilar Insufficiency and Posterior Circulation Stroke

Vertebrobasilar insufficiency leading to posterior circulation infarcts caused by congenital hypoplasia of the bilateral transverse foramina at the C2 level, affecting the caliber and flow of the bilateral distal cervical vertebral arteries in an adult, has not been previously reported. A 41-year-old male presented with episodic dizziness for a period of 1 year prior to consultation. Computed t...

Arterial pathology in cerebellar infarction.

We reviewed 88 pathologically proven cerebellar infarcts, examining the entire thoracocervical arterial system (including the spinal part of the vertebral artery, the intracranial arteries, and the heart) in all cases. For 50 infarcts, we found an arterial occlusion. The intracranial part of the vertebral artery was involved in 38 infarcts (76%), the basilar artery in 20 infarcts (40%), and a c...

Bilateral distal upper limb amyotrophy and watershed infarcts from vertebral dissection.

BACKGROUND Vertebral artery disease may give rise to lower motor neuron deficits, but the pathogenesis is unknown. I describe a man with a right vertebral artery dissection who developed bilateral distal upper extremity amyotrophy. He had symmetrical bilateral focal hyperintensites of the anterior cervical spinal cord on magnetic resonance imaging, compatible with watershed infarction. CASE D...

Vertebral artery insufficiency secondary to trauma and osteoarthritis of the cervical spine.

Obstruction of the vertebral arteries can lead to sudden death, extensive spinal cord destruction, insufficiency of the brain stem, impaired cerebellar function, damage to the occipital cortex, and may well be responsible for certain forms of cerebral palsy. We have recently treated a case of post-traumatic vertebral artery insufficiency by debridement of an osteophyte obstructing the intraspin...

Embolic brain infarction related to posttraumatic occlusion of vertebral artery resulting from cervical spine injury: a case report

INTRODUCTION The frequency of vertebrobasilar ischemia in patients with cervical spine trauma had been regarded as low in many published papers. However, some case reports have described cervical spine injury associated with blunt vertebral artery injury. Many aspects of the management of vertebral artery injuries still remain controversial, including the screening criteria, the diagnostic moda...
